The Warfarin/Anticoagulation Clinic in the Mater Hospital is a service that provides helpful information, monitoring and Warfarin dosing to a large number of patients who take anticoagulation medicine for a number of conditions.
We monitor patients on warfarin/Anticoagulation therapy for many different reasons, including:
We give patients their warfarin dose according to their individual target range, using the computer-assisted dosing (DAWN AC) system. Patient records are kept on our system and each patient also keeps their own record in their printed yellow record book.
At your first appointment at the clinic, we will talk to you about your Warfarin therapy, answer any questions you may have and we'll also give you helpful information on your therapy that you can take away with you. We will also provide you with ongoing information throughout your therapy.
Patients come to us for information and Warfarin therapy monitoring. We also provide each patient with their individual Warfarin doses, and we print the details of their dosage into their yellow record book. We have educated some patients to self-test their international normalised ratio (INR) using a CoaguChek XS in range machine. This is a blood test that checks how long it takes for blood to clot. The patients contact the clinic with the results from their self-test and we dose them accordingly.
The clinic also provides a DOAC nurse led clinic providing medication management monitoring and review.
All of our staff regularly attend anticoagulation conferences and are members of the special interest group CLOT (Clinical Leaders of Thrombosis). Our team has attended Birmingham University/Warwick University for anticoagulation training. We have contributed to an information poster called “Know Your Vitamin K”, which has been distributed nationally.
We only accept referrals from consultants within the hospital. We don’t accept direct GP referrals.
We will contact you with the date and time of your first appointment. Subsequent appointments will be printed into your yellow record book, along with your warfarin doses.
